The following documentation must be received and verified by the Nurse Providers' staff before we can place you on your first shift:

• Current CA Nursing License

• Current CPR

• Current Fire & Life Safety Card

• Current Identification document (CA ID card/Drivers License/Green Card)

• Social Security Card

• Recent Physical Exam (within the last 12 months)

• Proof of negative PPD/Chest X-ray

• Proof of Immunization for Measles/Mumps/Rubella (MMR) or Titer

• Proof of Immunization for Varicella or Titer

• Proof of Immunization for Hepatitis B or Hepatitis B Waiver

• Registry Application for Employment

• RN Skills Checklist (M/S)

• Critical Care Skills Checklist

• LDRP Skills Checklist

• Meds Skills Checklist

• NICU Skills Checklist

• Operating Room Skills Checklist

• Psychiatric Skills Checklist

• Telemetry Skills Checklist
